POLICIES OF LEAN LIFESTYLE, LLC
Lean lifestyle strongly advises that all clients consult their physician before participating in any dietary and/or exercise program. Lean Lifestyle expressly disclaims any and all implied warranties. Lean Lifestyle does not guarantee any effects as advertised as individual results vary based upon numerous factors.
Lean Lifestyle is not responsible for any food borne illness or sickness that may result from consumption of our products. All meals delivered Monday through Friday are meant for consumption with in two days of delivery. Meals delivered on Fridays are for Friday, Saturday and Sunday.
Lean Lifestyle requires that all meals be refrigerated immediately upon delivery thereof and not later than 9:00 a.m. on the day of delivery. Lean Lifestyle further requires that meals be delivered to a location with adequate shade for such time that meals remain outdoors and are not refrigerated, and that if taken away from the delivery address all meals are repackaged with fresh, frozen ice packs or ice to keep them from spoiling.
Once your personal eating plan has been established, we will prepare and deliver your meals on an ongoing basis. We will continue to provide this service until we are given appropriate notice of a change. You must notify us of any changes, (including changes in meal schedule, personal profile , start or stop dates, individual meal cancellations, additional meal requests and delivery location changes), by the Wednesday prior three-days prior to the week that the requested change be implemented. Please remember, all changes must be confirmed with your plan advisor before the deadline.
PAYMENT – A minimum order of $19.99 applies per scheduled delivery date to LEAN Lifestyle food purchases. If an order does not meet this minimum you will be charged the difference between the minimum daily order and your order. LEAN Lifestyle accepts payment by credit card only (Visa, MasterCard, or American Express). LEAN Lifestyle does not accept any other form of payment. Billing will occur the Wednesday prior to the week of upcoming scheduled deliveries. Billing will continue unless notified of a change prior to the deadline.
DELIVERY - Delivery is included at no cost in all meal plans where the average per delivery value exceeds $19.99 and the delivery location is within the delivery area disclosed herewith. Additional delivery charges apply for outlying areas. Each delivery is made overnight by 7:00 AM. Specific delivery times may be requested, however, LEAN Lifestyle does not guarantee that a requested delivery time will be granted. Meal deliveries may be delayed beyond the 7:00 a.m. delivery deadline in the event of factors outside of Lean Lifestyle’s control, such as acts of God, strike, or any other unforeseen circumstance not in Lean Lifestyle’s control. A Lean Lifestyle cooler will be delivered along with your order. Please put it, and the accompanying ice blocks, out for pick up on your next regularly scheduled delivery day. Failure to do so will result in a charge to your credit card in the amount of $54.95 plus tax per bag. LEAN Lifestyle will need the appropriate keys and/or entry codes to all delivery locations prior to the first day of service. LEAN Lifestyle guarantees that all modes of entry shall be kept strictly confidential. Meals are delivered to home/apartment addresses only. LEAN Lifestyle requires two business-days notice for address changes. LEAN Lifestyle reserves the right to implement a fuel surcharge (for delivery vehicles) in the event of inflated gasoline prices. |
